A Case Report of Radiation Induced Ulcer with Sacral Subcutaneous Calcification Following High-Dose Radiotherapy

Abstract
Radiotherapy is considered as a important therapeutic modalities in cancers. However, the deleterious effects of radiation may have a significant effect on local tissues. The vascular damage and fibrosis are well known late complications of radiation therapy. However, subcutaneous calcification is a very rare complication of radiation therapy. Here we report a case of radiation induced ulcer with sacral subcutaneous calcification. An 87-year-old woman admitted to our department with nonhealing sacral ulcer. She underwent postoperative high dose irradiation for uterine carcinoma thirty years ago. Physical examination revealed the infected ulcer in the sacral area measuring approximately 1.0¡¿1.0 cm. On investigation, her renal function, calcium, phosphate, parathyroid hormone and vitamin D level was normal. A cross-sectional computed tomographic scan of the pelvis showed a amorphous calcification located in the subcutaneous area of left sacral area. Wide excision and biopsy revealed chronic ulcer and calcium deposits in the dermis and subcutaneous tissue. Subcutaneous calcification is a uncommon sequela of fractionated radiotherapy. We report a rare case of sacral subcutaneous calcification after high dose radiation therapy. (J Korean Wound Management Soc 2010;6:77-79)
